ROLLOVER & TRANSFER OF SERVICES

Services are redeemable 60 days after the original debit date of the unused service(s). After 60 days, unused service(s) are forfeited. We will not grant refunds for unused service(s). Services are non-transferable.

 

APPOINTMENT CANCELLATION POLICY

You may cancel or reschedule an appointment with no charge 24-hours preceding your appointment time.  Same day cancellations or appointment changes will be charged a $15 late change/cancellation fee.  If you do not call to cancel or do not show up for your scheduled appointment, you will be charged the full price for the scheduled service.

 

MEMBERSHIP TERMINATION TERMS

A required minimum of two months on plan. You may initiate termination of your membership at any time by providing a thirty (30) day written notice before the next debit date.  Upon cancellation of your membership, all unredeemed membership services will expire thirty (30) days after the final membership payment has been processed.
